Patient concern

A patient in their late 30s presented after chipping the lower edges of the upper front four teeth on a hard food some years previously. The chips had become more uneven over time as further small fractures had occurred. They were self-conscious about smiling in photographs and at work, and had been avoiding biting into anything firm. They had previously been advised that crowns or veneers were the only options and were apprehensive about drilling healthy teeth.

Treatment provided

At consultation we discussed three options: composite bonding (additive, conservative, same-day), porcelain veneers (longer-lasting but requires enamel removal), or alignment first if the underlying tooth position was contributing to the wear pattern. Photographs and a digital scan were taken. The patient chose composite bonding for its conservative nature and same-day result. A 90-minute bonding appointment was scheduled. Each upper front tooth was lightly etched, primed, and built up in tooth-coloured composite layered to match the existing shade. Edges were sculpted and polished to a natural lustre. A custom night guard was prescribed because subtle grinding was identified during examination. Total cost: 4 teeth at £333 each = £1,332.

Timescale

From consultation to completed bonding: 2 weeks (consultation, then 90-minute bonding appointment). 2-week review confirmed the bite was settled. Patient returned for a polish at the 6-month hygiene visit; bonding still looking fresh at 12 months.
