Patient profile and presentation

The patient is a 41-year-old male, dentally well, no caries, sound restorations only, no periodontal concerns. He presented with intrinsic grey-banded staining of the upper anterior teeth — a textbook tetracycline-staining pattern caused by antibiotic exposure during tooth formation in childhood. The lower anteriors were similarly affected but less visible in his speaking and smiling pattern.

The patient had previously tried over-the-counter and dentist-supervised whitening with no meaningful improvement — predictable, because tetracycline staining is intrinsic and does not respond to peroxide-based bleaching. He now wanted a definitive cosmetic solution but was clear he could not commit to porcelain veneers given the cost.

Patient priorities: visible improvement of the upper anterior teeth, treatment delivered in one or two visits if possible, total cost not to exceed £3,500, and reversibility (no permanent removal of healthy tooth structure).

Clinical assessment and decision-making

Three realistic options were costed:

Option 1 — Composite bonding (8 upper teeth, UR4–UL4). £2,664 at Tower Dental (£333 × 8). Single-day treatment. Reversible (no significant tooth reduction). Expected longevity 5–7 years before refurbishment likely needed.

The patient elected Option 1 — composite bonding of 8 upper teeth — with the explicit understanding that intrinsic tetracycline staining is challenging to mask completely with composite alone, and that subtle banding may remain visible under bright direct light. This was demonstrated at the consultation with a single-tooth mock-up at no charge.

Treatment provided

Visit 1 — Consultation (£40, credited). Photographs, shade selection, single-tooth bonding mock-up to demonstrate the achievable masking and set realistic expectations. Patient consented to proceed.

Visit 2 (week 2) — Bonding day. Treatment was delivered across one full-day appointment (5 hours scheduled). Process:

1. Pre-treatment cleaning of all 8 teeth. 2. Application of an opaque-modifier base layer to mask the underlying tetracycline staining. 3. Body and incisal-layer composite placed in a free-hand layered technique. 4. Sculpting of contour, embrasures and incisal edges per the agreed shade and shape design. 5. Articulation and occlusal adjustment. 6. Polish and final glaze.

Total chair time: 4 hours 40 minutes. The patient left with the final outcome that day — no laboratory work, no temporaries, no second visit.

Visit 3 (week 4) — Follow-up review. No adjustments needed. Patient self-reported satisfaction 9/10. Photographic record at 4 weeks; comparison with pre-treatment photographs shows the masking effect.

Outcome and measurable data

Total chair time: 4 hours 40 minutes (single visit). Total elapsed time: 4 weeks from consultation to satisfied review. Number of teeth treated: 8 (UR4 through UL4). Patient self-reported satisfaction at 4-week follow-up: 9/10. Aesthetic outcome: banded staining substantially masked under typical lighting; subtle banding remains visible under direct bright light per the consultation discussion. Patient confirmed this was within the expectation set. Tooth reduction during preparation: 0.0mm. The bonding was placed additively over existing tooth structure with no preparation; the work is fully reversible if removed.

Aftercare and long-term plan
